Transcript Evaluation Procedures
Application Process
In order to qualify for transcript evaluation, an applicant must meet the following criteria:

If applying for a teaching certificate please complete an "Application for Teaching Certificate" (form 73.03C), appropriate for the type of certificate being sought:

If applying for an Administrative or School Service Personnel certificate please complete an "Application for School Service Personnel or Administrative Certificate" (form 73.03D), appropriate for the type of certificate being sought

Submit a non-refundable fee for each certificate requested to your Regional Office of Education. Please contact your regional office to determine who the fee should be made payable to, and the appropriate amount.

Submit official transcripts from all colleges attended. To be official, transcripts must be submitted in the sealed envelope from the college or university or be sent directly by the institution. Transcripts received that are not in a sealed envelope from the university or sent directly from the university will be considered unofficial and cannot be used for evaluation purposes.

Submit a signed and sealed State Approved Program Verification Form (73-05) completed by the institution where the approved program was completed.

Submit a copy of both sides of a comparable, valid, out of state certificate.

Submit any original letters of teaching experience from your employer(s). Letters need to state the grade level/subject and number of months of full-time teaching experience and must be signed by a district administrator or superintendent.

If you hold a valid, comparable, out of state certificate and wish to receive an Illinois Provisional Certificate, then submit a completed "Request for Issuance of a Provisional Certificate" (form 73-63)
Upon receipt of your certificate, you will be notified of the amount of the registration fees that you must submit to your regional superintendent's office.
