Program Participation Request Form for School-Based Child Nutrition Programs
Thank you for your interest in becoming a sponsor of a school-based child nutrition program! School-based child nutrition programs include the following: National School Lunch, School Breakfast, Afterschool Care Snacks, Special Milk, and Illinois Free Lunch and Breakfast.
Prior to completing the Program Participation Request Form for School-Based Child Nutrition Programs, prospective sponsors are required to become familiar with all of the program regulations and policies regarding program administration and operation. Be sure to review the Administrative Handbook and Monitoring and Program Requirements.
After reviewing all program regulations, policies, and other relevant information, complete the program participation request form and mail it along with all required documentation as noted on the form.
