GUIDE TO USING THE VERIFICATION SUMMARY FOR THE 2002
SCHOOL REPORT CARDS
Instructions: Please use this guide in verifying
the accuracy of the school report card data as reported to the State Board
of Education on the School Report Card Data Form (86‑43). Make
all corrections on the summary forms and return them to the Data Analysis
and Progress Reporting Division, Illinois State Board of Education, 100
N. First Street, Springfield, IL 62777, no later than August 22, 2003.
RETURN
THE VERIFICATION SUMMARY ONLY IF YOU HAVE CORRECTIONS TO BE MADE. CORRECTIONS
MUST BE RECEIVED BY AUGUST 20, 2003. UNLESS YOU SUBMIT CORRECTIONS, WE
WILL ASSUME THAT ALL INFORMATION APPEARING IN THE VERIFICATION SUMMARY
IS CORRECT. YOU MAY MAIL OR FAX (217-524-7784) YOUR CORRECTIONS.

Formulas and/or examples
are provided for most of the data elements to help you verify information
from your schools. You need verify only the actual counts; we will recalculate
the statistics if you make corrections in the counts. Please make all
corrections in red ink. If you FAX a correction, please do not send
a duplicate copy by mail.

There is a Page A for each of your schools. Page A
contains the data reported on the School Report Card Data Form (Form 86‑43)
for Items 1 through 12. Please refer to your copy of Form 86‑43
for each one of your schools as you review these data.
The low‑income count, student enrollment data
and limited-English-proficient (L.E.P.) count were reported on the Fall
Enrollment and Housing Report (Form 87-03) in October 2001.
NAME OF SCHOOL AND DISTRICT
Unless you change the spelling, order, or spacing,
the school and district names will appear on the report card as shown
on the summary. The grades served are taken from the Fall Enrollment
and Housing Report.

You will have one Page B which contains district level
data generated from the Teacher Service Record and Fall Enrollment
and Housing Report that were filed by the district in October 2001.
These data are provided for your information.
All teacher statistics on this page, except for the
pupil‑teacher ratio, include classroom teachers (position codes
18, 19, 20 and 22 from the Teacher Service Record). Pupil‑teacher
ratio does not include special education teachers (position code 18).
Only certified staff with administrative time listed in column 24 on the
Teacher Service Record are included in the administrator statistics.
Administrator and teacher salaries include tax sheltered
annuities, flexible benefit plan w/cash option, contributions to deferred
compensation plans, bonus payments, retirement incentives in addition
to the extra duty pay, and board paid retirement.
SUPERINTENDENT'S SIGNATURE AND DATE
The Superintendent's signature is required before changes
will be accepted.
