Individualized Education Program (IEP)

The Illinois State Board of Education's recommended Individualized Education Program (IEP) forms are in English and Spanish.

The packet includes the IEP instructions and the IEP forms in both PDF and word format. Additionally, a copy of the Summary of Performance is available.

The IEP forms can also be viewed individually.
